NOTE: Liability waivers must be filled out for minors, under the age of 18. Forms for a minor must be entirely filled out and signed by a legal guardian. If you are printing the waiver at home and sending it with your child, the child must also bring a copy of the guardian’s valid government ID. Also each day ticket requires a newly signed liability waiver.
